Claim Instructions

Use the number printed at the top of your Spyglass Investigations letter. Enter it exactly as shown, including all letters and numbers.
Review the Abandoned Property Recovery Agreement.
Confirm your name and contact information, complete every required field, and review the State-prescribed agreement before signing.
Sign, date and return it.
Return the completed form using the instructions provided in your letter. Keep a copy for your records.
Spyglass Investigations files and manages the claim.
We prepare the filing, organize supporting documents and handle Department follow-up through the review process.
The Department reviews the claim.
If the claim is approved, payment and any authorized representative fee are handled according to the signed Abandoned Property Recovery Agreement and the Department’s claim process.
No upfront recovery fee is collected at intake.The State-prescribed Abandoned Property Recovery Agreement discloses the authorized fee, costs and claimant net amount; fees and costs are owed only after an approved claim.
